Chronic Wound Consumables Scheme

The Chronic Wound Consumables Scheme funds wound care products for people with diabetes and a chronic wound who are aged 65 and over, or 50 and over for First Nations people.

Eligible health professionals assess the wound and order the necessary products via an online portal, choosing from a range of approved items.

These products are then delivered directly to the patient’s home or the health professional’s clinic—at no cost to the patient—ensuring timely access to the care they need.
