Yana – a healing journey for heart and stroke

Australian General Practice Accreditation Limited (AGPAL), in partnership with the Stroke Foundation, is leading Yana – a project supporting culturally safe, coordinated care for Aboriginal and Torres Strait Islander peoples recovering from heart disease and stroke.

Funded by the Australian Government Department of Health, Yana offers five micro-learning modules designed to strengthen frontline health workers’ ability to deliver culturally responsive care.

Living Stronger Longer chronic disease resources

The Living Stronger Longer Resource Kit supports health professionals working with First Nations clients managing chronic conditions.

Developed to be practical and culturally appropriate, the kit:

  • takes a holistic view of chronic disease

  • recognises the vital role of Aboriginal Health Workers

  • uses minimal medical jargon

  • supports conversations about multidisciplinary care and appointment attendance
