EOI open: Winter Strategy 2026

Following the success of previous Living Well Your Way (LWYW) winter activities, MPHN is inviting General Practices across the region to participate in Winter Strategy 2026.

Winter Strategy 2026 supports proactive and coordinated care for patients with chronic disease who may be at increased risk of deterioration during the colder months. The initiative aims to strengthen preventative care, reduce avoidable hospital use, and improve patient wellbeing.

Objectives of Winter Strategy 2026

  • Provide targeted and coordinated care for people with chronic disease at risk of increased health instability over winter

  • Reduce avoidable emergency department presentations and/or hospital admissions

  • Reduce mortality and morbidity for ‘at risk’ participants

How to apply

More information about the activity and eligibility criteria is available in the Expression of Interest (EOI) Guidance Document.
