Aged Care Outreach Service now available in Griffith, Leeton and Narrandera

Murrumbidgee Local Health District's new Aged Care Outreach Service (ACOS) is supporting older people in residential aged care and the community to access timely specialist assessment and care closer to home.

The service provides virtual and face-to-face clinical support, care coordination and specialist geriatric input to help manage acute and sub-acute health concerns, avoid unnecessary hospital presentations, support early discharge and improve patient outcomes.

Referrals are coordinated through the MLHD Centralised Care Intake Service (CCIS). The service is currently available for older people living in residential aged care in Griffith, Leeton and Narrandera, with Wagga Wagga and Deniliquin coming soon.
