Homelessness Health Support Team Program
The Homelessness Health Support Team Program supports people who are experiencing homelessness, or at risk of experiencing homelessness to access the healthcare they need. This may include people who are sleeping rough, couch surfing, living in temporary accommodation or staying in overcrowded housing.
Through the program, participants are connected with a care coordinator, service navigator or care worker who will work with them to understand their health and wellbeing needs, set individual goals and link them with the right healthcare and community supports.
Support may include help to plan and manage healthcare, understand health and wellbeing needs, navigate the healthcare system, access community-based services, connect with local support networks, and apply for relevant government funding or assistance.
The program is available in Wagga Wagga, Griffith, Junee, Hilltops, Greater Hume Shire and Edward River.
GP referrals are not required. Anyone can make a referral by contacting the Homelessness Health Support Team.
