The Living Well initiative has been established to change what we all suspect is a system in desperate need of redesign. We’re here to build a better way of doing things – a joined-up, interdisciplinary approach that focuses on better outcomes for people living with chronic conditions.
A joint initiative between Murrumbidgee Primary Health Network (MPHN) and Murrumbidgee Local Health District (MLHD) funded by the New South Wales Ministry of Health, Living Well is designed to streamline patient care for those living with Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF).
By changing the way we deliver healthcare, our goal is to improve the likelihood of people effectively and sustainably self-managing their chronic conditions and keeping them away from unnecessary hospital stays. Put simply, the Living Well, Your Way care pathway will help people navigate their way through the system, ensuring they have access to the personal, professional and community resources they need – when they need them, in the way they choose to use them – to live healthier, happier lives. It will also enable health care professionals to take a team approach and provide a more targeted level of care for their patients.
