The Outreach Heart Failure Diagnostic Clinics aim to provide timely and affordable access to early detection, assessment and management of heart failure, improving the quality of life.
Comprehensive heart failure care close to home
These clinics offer community-based screening, assessment, and diagnostic testing for patients identified by their GP as being at elevated risk or experiencing symptoms of heart failure. Referred patients receive:
A comprehensive cardiac assessment
Echocardiogram (ECHO) testing for heart function evaluation
Self-management support and education
Specialist follow-up to ensure ongoing care
Ongoing care is coordinated with the patient’s local GP, providing education and guidance on optimal heart failure management.
Clinic Locations & Specialist Support
Clinics are held twice a month at local general practices and Aboriginal Medical Services across the region. These clinics are supported by:
MLHD Clinical Nurse Consultants
Sonographers and private cardiac physiologists
Cardiac Advanced Trainees and Cardiologists (in person or via telehealth)
Delivered in partnership with Roy Cardiology and Riverina Cardiology, the program is funded under COAG 19(2) and facilitated through Living Well, Your Way.
