Universal Aftercare

The Universal Aftercare suicide prevention model provides support to people most at‐risk of suicide. This support assists a person to stay safe and connect with existing health and community services during a period of high risk and vulnerability.

Eligibility: People who have recently attempted suicide or those experiencing suicidal ideation at risk of suicide.

Location: MPHN region. Support Coordinators and Peer Workers are located in, or outreach to, all communities in the Murrumbidgee region. This support extends to anyone living in or accessing healthcare services in the Murrumbidgee.

How to access the service: The Murrumbidgee Local Health District (MLHD) Community Mental Health Teams, Inpatient Units and Mental Health Emergency Consultation Service (MHECS) are able to refer people on discharge from hospital to the service.

Referrals can also be made by General Practitioners, schools, aged care providers, and Headspace without requiring a hospital admission.
