Well Connected

Well Connected provides support to manage day to day needs such as:

  • Improving relationships with family and others

  • Maintaining physical wellbeing

  • Daily living needs

  • Education and training goals

  • Building broader life skills, including confidence and resilience

Eligibility: For people aged 16+ who experience severe mental illness and are not being supported by the National Disability Insurance Scheme (NDIS) funding.

Cost: All services are fully funded.

Provider: Wellways Australia.

Location: MPHN region.

Support workers are located in, or outreach to, all communities in the Murrumbidgee region.

How to access the service: Referrals to Well Connected can be made from clinicians, other support services, general practitioners, family members, friend or self.
