MPHN has released a new resource to help your general practice make manageable changes in line with the new GP Chronic Conditions Management (GPCCM) MBS items.

The Chronic Conditions Management Quality Improvement Workbook is designed to support quality improvement initiatives in general practice. It includes practical tips, examples, templates and activities to help you implement and embed the GPCCM items and MyMedicare Voluntary Patient Registration.

The workbook provides a step-by-step approach to:

  • Understand, implement and evaluate the CCM initiative within your practice

  • Improve coordination between general practice teams, multidisciplinary teams, patients and carers

  • Achieve measurable and sustainable improvements to models of care

Designed to be practical and flexible, the workbook allows you to pick and choose the sections and activities that are most relevant to your practice.
