Primary Care Nurse & Clinical Programs Manager

We’re seeking an engaging and motivated Primary Care Nurse & Clinical Programs Manager to provide clinical leadership across general practice, with a strong focus on nurse‑led models of care, immunisation programs and primary care workforce capability.
This is a highly visible role where you’ll represent MPHN across the community - partnering with general practices, collaborating with health system stakeholders and supporting high‑quality, evidence‑based care across the region.
About the role
Reporting to the Senior Manager Integration, you will:
Provide clinical leadership and practice‑based support to strengthen nurse‑led models of care in general practice.
Lead and support immunisation systems, workforce capability and alignment with National and State schedules.
Support delivery of priority clinical programs responding to population and system needs.
Provide timely, evidence‑based advice on infectious and vaccine‑preventable diseases.
Contribute to clinical governance, quality improvement and safety.
Play a key role in emergency preparedness and response, supporting primary care providers when needed.
Build strong, trusted relationships with GPs, nurses, pharmacists, Local Health Districts and community partners.
About You
You’re a personable and confident clinician who enjoys working closely with GPs, nurses and the wider community. You bring a collaborative, relationship‑focused approach and are comfortable representing your organisation across a range of professional and community settings.
You will have experience in primary care or community health, with a sound understanding of immunisation delivery and vaccine‑preventable diseases. You communicate clearly and professionally, both in writing and verbally, and enjoy supporting clinicians to deliver high‑quality, evidence‑based care.
Experience in general practice nursing, primary care workforce development, Primary Health Networks or emergency response activities is highly regarded.
