Back on Track Physiotherapy, proudly funded by Murrumbidgee Primary Health Network (MPHN), is excited to bring you the Vitality Passport – a program designed to help you take control of your health and live life to the fullest.
Vitality Passport gives you the tools, strategies, and support you need to improve your strength, mental health, nutrition, and cognition, so you can stay active, independent, and doing the things you love for longer.
What is the Vitality Passport?
The Vitality Passport is an evidence-based, proactive health program that aims to:
Improve strength and balance
Support mental health and wellbeing
Improve nutrition and reduce muscle loss
Boost cognitive function
Encourage active community participation
We want you to feel stronger, think sharper, and live longer — all while enjoying a happier, more connected life.
What’s Involved?
One-on-One Consultations: You'll receive three personalised consultations (initial, review, and discharge planning) with each of the expert clinicians. Appointments are conveniently offered at the same time, in the same place to make it easy for you.
Building Physical Reserve (Physiotherapy/Exercise Physiology): Clinicians will design an exercise plan tailored to you — whether you’re just getting started or ready to step it up. They focus on prescribing the right amount and type of exercise to build strength, resilience, and improve quality of life.
Building Nutritional Reserve (Dietetics): Dietitians work with you to fine-tune your diet, focusing on protein intake and timing to prevent muscle loss and boost strength. Small changes can make a big difference!
Building Cognitive Reserve (Occupational Therapy): Just like your body, your brain needs exercise too! Clinicians help you engage in brain-stimulating activities that align with your interests, helping you build cognitive reserve for a healthier mind.
Supporting Mental Health and Wellness: Mental health clinicians guide you through wellness strategies to support your mental health today and into the future. Early support can make all the difference.
Group Activities: Vitality Passport also offers 9 engaging group sessions where you’ll share experiences, build friendships, learn about healthy living, and stay active and connected. Group activities are a fantastic way to boost wellbeing while having fun!
