Chronic Conditions Prevention and Integrated Care Grants Program

The Chronic Conditions Prevention and Integrated Care Grants Program is now open, offering competitive funding for initiatives that improve health outcomes for people living with, or at risk of developing, chronic conditions.

The program aligns with the National Strategic Framework for Chronic Conditions 2026–2035 and supports activities that strengthen prevention, early detection, integrated and person-centred care, and health equity.

The Australian Government has committed $109.9 million over three years from 2026–27, with ongoing funding of $38.3 million per year from 2029–30 to support the program.

Funding will be available through two tiers:

  • Tier 1 – Larger, multi-year national initiatives, typically delivered through consortiums or partnerships, addressing multiple chronic conditions or shared risk factors

  • Tier 2 – Smaller, time-limited activities delivered by individual organisations to implement targeted projects or pilot initiatives
