Supporting joined-up care through HealthPathways

Looking for the right referral options for your clients, or interested in being included in local referral pathways?

HealthPathways is a free, online decision support tool designed for healthcare professionals to use at the point of care. It provides guidance on assessing and managing medical conditions, along with information on referring patients to local specialists and services in a timely way.

We are reviewing and updating allied health provider listings in HealthPathways as part of our ongoing work to increase awareness of the services you provide and to streamline referrals from health professionals across the region.

Over the coming months, a member of our team may contact you about your HealthPathways listing. A prompt response will help ensure your service information is accurate and up to date when clinicians are seeking referral options.

To be included in HealthPathways, we need information about your service, including referral processes and eligibility criteria (including inclusion and exclusion criteria). If this information is available on your website, it can help streamline the process.

What you can do

  • Check your website to ensure referral processes and eligibility criteria are current and clearly outlined

  • If contacted by our team, please provide the requested information as soon as possible to support timely and accurate inclusion in HealthPathways

Register or sign in to Murrumbidgee HealthPathways, or contact the team via email at HealthPathways@mphn.org.au.
