EOIs open: Living Well, Your Way - Winter Strategy 2025

MPHN in conjunction with the Living Well, Your Way (LWYW) Initiative, are seeking Expressions of Interest (EOI) from General Practices within MPHN region to participate in Living Well, Your Way - Winter Strategy 2025. 

The purpose of LWYW - Winter Strategy 2025 is to support proactive care for patients with chronic disease, at risk of deterioration over the winter period.
