Support at Home: a quick guide for GPs
The Support at Home program is now the main pathway for older patients to access in-home care, replacing Home Care Packages.

End-of-Life Pathway
For patients who wish to remain at home in their final months:
Eligible if life expectancy is ≤3 months and AKPS ≤40
Provides short-term funding for care at home
Requires GP completion of the pathway form
