Support at Home: a quick guide for GPs

The Support at Home program is now the main pathway for older patients to access in-home care, replacing Home Care Packages.

what you need to know

Support at Home helps with:

  • Clinical care such as nursing and allied health

  • Independence support including personal care and transport

  • Everyday living such as meals, cleaning and shopping

Patients are assigned a funding level based on need, with short-term pathways available for specific situations.

  • Refer via My Aged Care

  • Escalate urgent cases where patients are at risk

  • Request a review if needs change

End-of-Life Pathway

For patients who wish to remain at home in their final months:

  • Eligible if life expectancy is ≤3 months and AKPS ≤40

  • Provides short-term funding for care at home

  • Requires GP completion of the pathway form
