Planning for future health care

We don’t always know what lies ahead, especially when it comes to our health. Advance care planning helps people think about what matters most to them and make their wishes known. If a person becomes unable to speak for themselves, advance care planning can help ensure their choices are understood and respected.

While advance care planning is not a core or expected part of the Care Finder role, Care Finders can support people to:

  • Think about what matters to them and the care they would and wouldn’t want

  • Talk about their wishes with trusted people and those involved in their care

  • Record their wishes in writing

  • Share their plan with the right people

  • Review their plan over time

Advance care planning can provide peace of mind and help ensure future decisions reflect what matters most to the individual.

Starting a conversation is a great first step.

Advance Care Planning Australia will host two Lunch and Learn webinars in July and August. Care Finders are invited to attend one session to learn more about advance care planning, how to support conversations, and available referral and support pathways.
