National Dementia Support Program survey

The Department of Health and Aged Care wants to hear about your experiences with the National Dementia Support Program, delivered by Dementia Australia.

Healthcare professionals and aged care workers who support people with dementia, their families, and their carers are encouraged to have their say.

All feedback is confidential and will contribute towards an independent evaluation of the Program.

The survey takes 10-15 minutes to complete and closes at 5pm AEDT, 29 March.
