Living Well Your Way

We are seeking support from your LHAC members to promote the following activities to your communities.   

Living Well, Your Way (LWYW) aims to build a better way of delivering healthcare for people with chronic heart (CHF) and lung (COPD) conditions to keep people healthier at home for longer.  

A full list of current activities and promotional resources about LWYW are available on the following webpages: 

  1. Pharmacy Screening and Education Program – Free assessments and referrals for anyone suffering symptoms of chronic respiratory or lung conditions. Plus free education for patients diagnosed COPD and CHF about their condition and medication provided in pharmacy. Click here for promotional material. 

  2. Heart Failure Diagnostic Outreach Clinics – Visiting specialists and clinicians visit rural communities to provide testing and diagnostics for patients suffering from symptoms of heart failure. Clinics welcome referrals from GP in nearby towns. Ask your GP today if you are eligible for a referral. To find out more click here.  

  3. Cardiac and Pulmonary Rehabilitation – FREE exercise and education programs available for people diagnosed with chronic heart and respiratory conditions in your area - Call CCIS 1800 654 324 for a referral. For more information click here

  4. Lungs in Action: Free community based exercise program for anyone living with a diagnosed chronic lung disease  – Find a class near you. Click here for more information.
