Team Care in General Practice
The service is intended to support people with severe, complex and persistent mental illness who access general practice for care and support.
Eligibility: The program is for people (16+ years) living in Federation, Berrigan, Junee or Cootamundra regions
Cost: All services are fully funded
Providers: Grand Pacific Health & Moving Tails
Location:
Grand Pacific Health: Cootamundra, Junee, Federation and Berrigan LGA’s within nominated general practices
Moving Tails: Snowy Valley LGA – Tumut and Tumba within nominated general practices
How to access the service – GP or Team Care clinician to conduct the IAR-DST to determine the persons level of care needs. IAR-DST Level of Care (LoC) 4 are appropriate to refer to Team Care.
