Team Care for People Experiencing Severe Mental Illness

Provides intensive psychological intervention and care coordination by mental health nurses. Given the complex needs of the target group, the service also supports better integration between social care and community supports, health and medical supports.

Eligibility:  People aged 16+ with severe and persistent mental illness, who are not actively supported by the Community Mental Health Team

Cost: All services are fully funded

Provider: Team Care for People Experiencing Severe Mental Illness is available throughout the MPHN area by Grand Pacific Health and Mururmbidgee Local Health District and also in selected general practices.

Location: Throughout the MPHN area as part of the MyStep to General Wellbeing program and selected general practices

How to access the service: Referrals can be sent via the MPHN Central Access and Navigation Service. Self referral are also welcome. A mental health treatment plan from a GP may be needed for ongoing support.
