Team Care for People Experiencing Severe Mental Illness

Team care for people with severe mental illness delivers intensive psychological interventions and care coordination by mental health clinicians under the MyStep to Mental Wellbeing program. Recognising the complex needs of the target group, the service also supports stronger integration across social care, community supports, and health and medical services.

Eligibility:  People aged 16+ with severe and persistent mental illness, who are not actively supported by the Community Mental Health Team

Cost: All services are fully funded

Provider: Team Care for People Experiencing Severe Mental Illness is available throughout the MPHN area by Grand Pacific Health and Murrumbidgee Local Health District.

Location: Throughout the MPHN area as part of the MyStep to Mental Wellbeing program.

How to access the service: Referrals can be sent via the MPHN Central Access and Navigation Service. Self referral are also welcome. A mental health treatment plan from a GP may be needed for ongoing support.
