Aboriginal Health – Integrated Team Care
Murrumbidgee PHN provides funding to local Aboriginal Medical Services (AMS) and Marathon Health Ltd to deliver the Integrated Team Care Program for Aboriginal and Torres Strait Islander People in our region.
Integrated Team Care activities are designed to increase the access to primary health care services by Aboriginal and Torres Strait Islander people.
The service offers Care Coordination support for people with a chronic disease, and general support to access health services through Outreach Workers.
To be eligible for the Care Coordination program Aboriginal and Torres Strait Islander patients must:
be referred by their General Practitioner
be enrolled for chronic disease management in a general practice or an Aboriginal Medical Service/Aboriginal Community Controlled Health Service
have a current GP Management Plan
have a chronic disease that has been, or is likely to be, present for at least six months
Aboriginal and Torres Strait people who visit a GP located in an AMS in the Murrumbidgee region (Riverina Medical and Dental Aboriginal Corporation in Wagga Wagga, Griffith Aboriginal Medical Service in Griffith, Murrin Bridge Aboriginal Health Service in Lake Cargelligo or Viney Morgan in Barmah) will be managed by the team at the relevant AMS.
Aboriginal and Torres Strait who visit a mainstream ‘non AMS’ General Practice, can be referred into the program to Marathon Health Ltd.

Aboriginal Cultural Orientation Training
This online Aboriginal Cultural Orientation package is designed for health professionals, health science students and people working in various health care settings. It aims to help you deepen your understanding of Aboriginal cultural values, beliefs and practices and subsequently enhance your delivery of culturally appropriate health services. It consists of five self-directed learning modules.
