Healthy ageing and palliative care
Supporting people to age well means helping them stay connected, informed and able to access the care they need while respecting their choices, experience and independence.
Across the region throughout 2025-2026, MPHN worked with older people, communities, providers and partners to tackle some of the practical challenges of ageing in a regional setting. This included redesigning primary care for residential aged care homes in thin markets, making services and support easier to navigate through the Wise + Well Seniors Expo, and challenging ageism by taking conversations about respect and inclusion directly into communities.
The result is a practical, locally informed approach that supports older adults to live well, maintain their independence and remain part of the communities they know.
