MBS Item Numbers for Vulnerable Patient Cohorts Including Case Conferencing

Join Kim Poyner as she provides an overview of the benefits of using case conferencing and MBS items available to provide care to the vulnerable population (living in the community, not those in residential care). GPs and Practice Nurses/Support staff will learn how to make time to ‘book in case conferencing’ to utilise item numbers and improve patient outcomes. You’ll also hear about the programs which focus on identifying health concerns within the community and providing support through referrals to allied health services.
