The General Practice in Aged Care Incentive program Guidelines are now available.
The General Practice Aged Care Incentive supports general practices to deliver regular, proactive services and care planning to older people living in residential aged care homes.
Services Australia have published information for practices about the process in PRODA and HPOS to register the practice to participate in the General Practice in Aged Care Incentive, and how to link Responsible Providers and patients.

Register now for the national webinar, 4 July from 11:00 am – 12:00 pm, introducing the General Practice in Aged Care Incentive with the Department of Health and Aged Care, in collaboration with the Australian Association of Practice Management (AAPM) and the PHN Cooperative. This session will introduce the General Practice in Aged Care incentive including an overview of the program guidelines.
