EOIs now open for General Practice Winter Strategy 2024

We're seeking expressions of interest from general practices to participate in the Living Well, Your Way – General Practice Winter Strategy 2024 program.

The General Practice Winter Strategy program provides general practices with additional funding for proactive care and support activities for patients with chronic disease who are at risk of deterioration over the winter period.

Practices involved in past programs valued this opportunity to improve their patients’ quality of life through proactive care and disease management, leading to a reduction in exacerbations and hospital presentations.

This year's program also offers the opportunity to concurrently complete two quality improvement cycles using HeartPlan in a box. HeartPlan in a box is an innovative QI Toolkit at the forefront of driving improvements in the management of heart failure patients within primary health care. The goal is to optimise outcomes for patients residing in our communities with heart failure. Find out more.

The General Practice Winter Strategy program is delivered by Murrumbidgee Primary Health Network in collaboration with Murrumbidgee Local Health District as part of the Living Well, Your Way Initiative. 

The EOI deadline has been extended until 5.00pm, Wednesday 20 March 2024. Please email your completed submission to tenders@mphn.org.au 

The General Practice Winter Strategy 2024 will commence in early April 2024.
