GP2.4 Our practice team transfers care to another practitioner (in our practice or in another practice) when a patient requests the transfer.

General practice must maintain a procedure for when a patient chooses to transfer their care to another GP, either internally or to another practice altogether. This decision needs to be documented in the patient’s health record with all relevant details, and the action taken. The patient’s health information must also be transferred to the new GP. Practices can further support the process by providing referrals to other healthcare providers.  

When a patient requests their care be transferred to the care of a practitioner in another practice, general practice must comply with state or territory laws on the transfer of patient health information.
