Perform kidney health check - Early detection of CKD offers the best hope for improving patient outcomes. A kidney health check should be done every 1 to 2 years for individuals with risk factors.
Chronic Kidney Disease Supportive Management
This pathway is about managing symptoms in adults with stage 5 chronic kidney disease i.e., GFR < 15, whether receiving kidney replacement therapy (dialysis or transplantation) or supportive care.
The aim of the services is to maintain kidney function, prevent disease progression and provide care, support and education.
